The State of Washington accepts the following filings via SERFF:
*Property/Casualty All TOI's accepted as listed on the Product Coding Matrix.
*Life, Accident & Health, Annuity and Credit Product Coding Matrix The following are accepted:
*ALL TOI's for Annuities, Credit, Life, Multi-Line and Viatical Settlements.
*Health - Limited TOI's, specifically:
H01 Health Assumption Agreement
H02G Group Health Accident Only
H02I Individual Health Accident Only
H03G Group Health AD&D
H03I Individual Health AD&D
H11G Group Health Disability Income
H11I Individual Health Disability Income
H19I Individual Health Travel
H19G Group Health Travel
Effective September 1, 2004, for the above Health TOI's indicated by Italics, the Rate portion of a filing (which includes the Actuarial Memorandum) MUST be filed separately, but simultaneously with the FORM filing. You will have TWO separate SERFF filings; one RATE filing and one FORM filing, i.e., policy, certificates, application, riders, endorsements, etc.
